Pediatric Dose

Pediatric dosing adjusts adult medication doses for children based on weight or age.

Clark"s Rule (weight-based, preferred): Child Dose = (Child Weight in lbs / 150) x Adult Dose

Or in metric: Child Dose = (Child Weight in kg / 68) x Adult Dose

Young"s Rule (age-based): Child Dose = (Age in years / (Age + 12)) x Adult Dose

Which rule to use:

  • Clark"s Rule is generally more accurate because it accounts for the child"s actual size
  • Young"s Rule is useful when weight is not available
  • Both are approximations — weight-based mg/kg dosing from drug references is always preferred

  • These rules are rough estimates for educational purposes
  • Always use manufacturer"s pediatric dosing guidelines when available
  • Never exceed the adult dose
  • Some medications have specific pediatric formulations with different concentrations
