Treadmill Incline to Outdoor Pace Calculator
Convert treadmill speed and incline to equivalent outdoor flat-road pace.

Running on a treadmill is not the same as running outside, and the difference goes beyond the belt moving under you.
On a flat treadmill at 0% incline, you do slightly less work than running outdoors at the same speed.
The belt assists your stride slightly, and you face no air resistance.
This is why most coaches recommend setting the treadmill to 1% grade to approximate outdoor effort on flat ground.
Every additional percent of incline adds meaningful extra effort.
The ACSM (American College of Sports Medicine) metabolic formula quantifies this precisely:
VO2 (ml/kg/min) = (0.2 × speed_m/min) + (0.9 × speed_m/min × grade_fraction) + 3.5
The equivalent flat-ground speed that demands the same oxygen consumption is:
equivalent_speed = (VO2_grade - 3.5) ÷ 0.2
Cancel the terms and the whole thing collapses to something you can do in your head:
equivalent_speed = speed × (1 + 4.5 × grade_fraction)
So every 1% of grade costs you about 4.5% of speed. At 8 mph (214 m/min) on a 5% grade, VO2 works out at 56 ml/kg/min, and the flat-ground speed that demands the same oxygen is 8 × 1.225 = 9.8 mph. That surprises most people, who treat incline as a gentle modifier.
Walking and running use different equations, and mixing them up is expensive
The formula above is the ACSM running equation, and it is only valid at 5 mph and above. Below that you are walking, which is a different movement with a different energy cost: walking is cheaper than running on the flat, and more expensive up a steep hill, because a runner recovers part of each stride from stored elastic energy while a walker lifts their whole body weight step by step. ACSM publishes a separate equation for it:
VO2_walking = (0.1 × speed_m/min) + (1.8 × speed_m/min × grade_fraction) + 3.5
The grade coefficient is 1.8 rather than 0.9, twice as steep, sitting on a flat-ground term that is half as large. Convert both to an equivalent flat running speed and the difference is easy to see:
- running equation: belt speed × (1 + 4.5 × grade)
- walking equation: belt speed × (0.5 + 9 × grade)
The two lines cross at a grade of 11.1%. Below that the running equation overstates a walk, and it overstates badly on the flat: walking 3 mph on level ground costs about what running 1.5 mph would, not 3. Above 11% it goes the other way and starts to understate. Walk 3.5 mph at 15% and the running equation says 5.9 mph while the walking equation says 6.5, a 10% shortfall that keeps growing with the slope.
That crossover is the real reason steep incline walking earns its reputation. A 12% walk at 3.5 mph is worth about a 5.5 mph run, which is a 10:51 mile, from a belt speed most people would call a stroll.
Between 4 and 5 mph you are in the awkward zone where some people run and some walk. Pick whichever you are actually doing; the calculator uses the running equation from 5 mph up.
