What Is Acetaminophen?

Acetaminophen (known as paracetamol outside North America; brand name Tylenol) is one of the world’s most widely used pain relievers and fever reducers. Unlike ibuprofen it does not reduce inflammation. It works by blocking pain signals in the brain and resetting the body’s temperature thermostat in the hypothalamus, which is why it does nothing for a swollen ankle and a good deal for the fever that comes with flu.

How It Works in the Body

Acetaminophen is metabolized primarily in the liver. At normal doses it is broken down safely and excreted. At high doses a toxic metabolite called NAPQI (N-acetyl-p-benzoquinone imine) is produced faster than the liver can neutralize it. NAPQI damages liver cells and causes acute liver failure, and acetaminophen is the leading cause of acute liver failure in the United States.

The unnerving part is how narrow the margin is. The maximum safe daily dose for a healthy adult is 4,000 mg and serious toxicity starts somewhere around 7,500 mg in one go. There is no other drug on a supermarket shelf where the safe dose and the dangerous one sit less than a factor of two apart.

Adult Dosing

Standard safe dose for healthy adults:

  • 325–650 mg every 4–6 hours as needed, OR
  • 1,000 mg every 6–8 hours (extra-strength)
  • Maximum per day (healthy adults): 3,000–4,000 mg
  • With liver disease or regular alcohol use: Maximum 2,000 mg per day

Pediatric Dosing

For children under 12:

  • 10–15 mg per kilogram per dose
  • Maximum per day: 75 mg/kg, or 5 doses in 24 hours, whichever comes first
  • Dosing interval: Every 4–6 hours
  • Common liquid concentrations: 160 mg/5 mL (children) or 32 mg/mL (infant drops)

Weight-based dosing stops at the adult ceiling. This is the part most dosing charts leave out, because charts usually stop at 45 kg (about 95 lbs). Run the arithmetic past that point and it breaks: 75 mg/kg/day crosses 4,000 mg at roughly 53 kg, and 15 mg/kg per dose crosses 1,000 mg at about 67 kg. Plenty of 11-year-olds weigh that much. A child never gets more than an adult, so a single dose is capped at 1,000 mg and the daily total at 4,000 mg no matter what the per-kilogram maths says.

The Hidden Acetaminophen Danger

Acetaminophen is found in hundreds of combination products: NyQuil, DayQuil, Theraflu, Percocet, Vicodin, Tramadol+Acetaminophen, and many prescription pain pills. Taking multiple products simultaneously is a common cause of accidental overdose. Always check every label for acetaminophen content and add up your total daily dose across all products.

Overdose Action

If you suspect an acetaminophen overdose, call Poison Control immediately: 1-800-222-1222 (US). Do not wait for symptoms. The antidote, N-acetylcysteine (NAC), works almost completely if it is started within 8 to 10 hours and its effectiveness falls away after that.

Waiting is the trap here, because a serious acetaminophen overdose feels like nothing much for the first day. Some nausea, maybe. The liver damage is well underway before anyone feels ill enough to go in, which is the opposite of how most poisonings behave and the reason the advice is to call on suspicion rather than on symptoms.
